Anastrozole, Letrozole, or Exemestane? A Clear-Eyed Patient's Guide to the Three Aromatase Inhibitors

A breast cancer diagnosis brings with it an overwhelming number of decisions, many of them unfamiliar and time-sensitive. For postmenopausal women with hormone receptor-positive (HR+) breast cancer, adjuvant hormonal therapy is a cornerstone of treatment — and aromatase inhibitors (AIs) are among the most widely prescribed agents in this category.

Three aromatase inhibitors are currently approved by the U.S. Food and Drug Administration for this indication: anastrozole (brand name Arimidex), letrozole (Femara), and exemestane (Aromasin). While all three work by reducing estrogen levels in the body, they are not interchangeable in every respect. Understanding their differences empowers you to ask better questions and engage more meaningfully in shared decision-making with your oncologist.

How Aromatase Inhibitors Work — and Why That Matters

In postmenopausal women, the ovaries no longer produce significant amounts of estrogen. However, estrogen continues to be synthesized in peripheral tissues — including fat, muscle, and the adrenal glands — through a process catalyzed by an enzyme called aromatase. In hormone receptor-positive breast cancer, estrogen can fuel tumor growth by binding to receptors on cancer cells.

Aromatase inhibitors work by blocking the aromatase enzyme, thereby reducing circulating estrogen levels and starving hormone-sensitive tumors of the signal they need to proliferate. This mechanism distinguishes AIs from tamoxifen, which blocks estrogen receptors rather than reducing estrogen production.

All three approved AIs accomplish this same fundamental goal — but they do so through slightly different chemical mechanisms, which has downstream implications for their side effect profiles and how individual patients respond.

The Key Pharmacological Distinction: Steroidal vs. Non-Steroidal

Anastrozole and letrozole are classified as non-steroidal aromatase inhibitors. They bind reversibly to the aromatase enzyme, inhibiting its activity without permanently altering its structure. When the drug is cleared from the body, the enzyme can resume normal function.

Exemestane, by contrast, is a steroidal aromatase inhibitor — sometimes called an aromatase inactivator. It binds irreversibly to the aromatase enzyme, permanently disabling it. New enzyme must be synthesized before estrogen production can resume. This distinction influences both potency considerations and certain aspects of the side effect profile.

Comparing Efficacy: What the Clinical Evidence Shows

Anastrozole (Arimidex)

Anastrozole has one of the most robust evidence bases of any adjuvant hormonal therapy. The landmark ATAC trial (Arimidex, Tamoxifen, Alone or in Combination), which enrolled more than 9,000 postmenopausal women with early-stage breast cancer, demonstrated that anastrozole was superior to tamoxifen in reducing disease recurrence over a five-year treatment period. Patients receiving anastrozole experienced a significantly lower rate of contralateral breast cancer and fewer thromboembolic events compared to tamoxifen users.

Subsequent studies have confirmed anastrozole's efficacy in both the early-stage adjuvant setting and in the treatment of advanced or metastatic HR+ breast cancer.

Letrozole (Femara)

Letrozole has been evaluated in the BIG 1-98 trial, one of the largest adjuvant hormonal therapy studies ever conducted. Results demonstrated that letrozole provided superior disease-free survival compared to tamoxifen over a five-year period, with particular advantages observed in women with lymph node-positive disease.

Letrozole is also approved for use in the neoadjuvant setting (before surgery) to reduce tumor size, and it has demonstrated activity in patients whose disease has progressed on anastrozole, suggesting some degree of non-cross-resistance between the two non-steroidal agents — though this remains an area of ongoing research.

Exemestane (Aromasin)

Exemestane's efficacy was established in the IES trial, which showed that switching to exemestane after two to three years of tamoxifen therapy significantly improved disease-free survival compared to continuing tamoxifen. This "sequential" approach informed treatment sequencing strategies that remain relevant today.

Head-to-head comparisons between all three AIs are limited, and no single agent has been conclusively demonstrated to be superior to the others in terms of overall survival across all patient populations. Oncologists typically consider individual patient factors — including comorbidities, prior treatment history, and tolerability — when selecting among them.

Side Effect Profiles: Where the Differences Are Most Clinically Relevant

All three aromatase inhibitors share a common side effect profile rooted in their shared mechanism: estrogen deprivation. Joint pain and stiffness (arthralgia), hot flashes, vaginal dryness, decreased libido, and an increased risk of osteoporosis are associated with each agent. However, there are meaningful differences in the frequency and severity of specific side effects.

Bone Health

Estrogen plays a critical role in maintaining bone density. All AIs accelerate bone loss to some degree, increasing the risk of osteoporosis and fracture over time. Baseline bone density assessment (DEXA scan) and ongoing monitoring are recommended for all patients on AI therapy.

Some studies suggest that exemestane may have a slightly more favorable bone profile due to its steroidal structure, which gives it weak androgenic properties. However, clinical guidelines recommend calcium and vitamin D supplementation, weight-bearing exercise, and bisphosphonate therapy (when indicated) for all patients on AIs, regardless of which agent is prescribed.

Cardiovascular Risk

Anastrozole has been associated with a modestly elevated risk of cardiovascular events in some studies, particularly in women with pre-existing cardiovascular risk factors. Letrozole and exemestane have also shown cardiovascular effects in various analyses, though the data across agents are not entirely consistent. Your oncologist will consider your cardiovascular history when selecting and monitoring your treatment.

Lipid Profiles

Letrozole has been associated with less favorable changes in cholesterol levels in some analyses compared to anastrozole, though the clinical significance of these differences varies. Women with pre-existing hyperlipidemia should have their lipid panels monitored during treatment.

Cognitive and Mood Effects

All three AIs have been associated with cognitive complaints — including memory difficulties and concentration problems — as well as mood changes such as depression and anxiety. These effects are believed to be related to estrogen's role in neurological function. Individual patients may find that one agent is better tolerated than another in this regard, though large-scale comparative data are limited.

Cost and Generic Availability

For many patients in the United States, out-of-pocket cost is a practical consideration that directly influences treatment adherence.

If cost is a barrier to adherence, it is important to discuss this openly with your oncologist or a patient navigator. Skipping doses to manage expenses can meaningfully reduce the effectiveness of your treatment.

If you are experiencing significant side effects on your current aromatase inhibitor, do not discontinue the medication without first speaking with your physician. Many side effects can be managed, and alternatives exist if your current regimen is genuinely not tolerable.

Hormonal therapy with an aromatase inhibitor is one of the most effective tools available for reducing the risk of recurrence in HR+ breast cancer. Understanding your options is not just an academic exercise — it is an investment in your long-term health and confidence in your treatment plan.
