For breast cancer treatment, this is the therapeutic goal. For bone health, however, this degree of estrogen suppression accelerates the very process that menopause had already set in motion.

Clinical research confirms the concern. Studies have shown that postmenopausal women taking anastrozole can lose bone mineral density at a rate significantly faster than women not receiving aromatase inhibitor therapy. The ATAC trial, one of the landmark studies evaluating anastrozole in early breast cancer, documented measurable reductions in bone mineral density at both the spine and hip over five years of treatment. Fracture risk rises accordingly—a fact that makes proactive bone health management not optional, but necessary.

Starting With a Baseline: The Role of DXA Screening

Before beginning anastrozole, or as early in treatment as possible, patients should undergo a dual-energy X-ray absorptiometry (DXA) scan. This imaging study measures bone mineral density and provides a T-score—a number that compares your bone density to that of a healthy young adult. A T-score between -1.0 and -2.5 indicates osteopenia (low bone mass), while a score below -2.5 meets the diagnostic threshold for osteoporosis.

The American Society of Clinical Oncology (ASCO) recommends that women initiating aromatase inhibitor therapy receive a baseline DXA scan, with follow-up scans typically performed every one to two years depending on individual risk factors. Your oncologist may coordinate this with your primary care physician or refer you to an endocrinologist or rheumatologist with expertise in metabolic bone disease.

In addition to DXA results, your care team may use the FRAX tool—a validated algorithm developed by the World Health Organization—to estimate your ten-year probability of a major osteoporotic fracture. This calculation incorporates age, body mass index, prior fracture history, smoking status, glucocorticoid use, and other variables to generate a personalized risk estimate that can guide treatment decisions.

Lifestyle Modifications That Make a Measurable Difference

No single intervention replaces the others, but a combination of evidence-based lifestyle changes can meaningfully slow bone density loss during anastrozole therapy.

Calcium and Vitamin D Intake Calcium is the primary mineral in bone tissue, and vitamin D is essential for its absorption from the gastrointestinal tract. The National Osteoporosis Foundation recommends that postmenopausal women aim for 1,200 mg of calcium daily from food and supplements combined, along with 800 to 1,000 IU of vitamin D3. Dairy products, fortified plant-based milks, leafy greens, and canned fish with bones are reliable dietary sources of calcium. Because many Americans—particularly older adults—have insufficient vitamin D levels, a blood test to check your 25-hydroxyvitamin D level before supplementing is advisable.

Weight-Bearing and Resistance Exercise Physical activity that places mechanical stress on the skeleton stimulates bone-forming cells called osteoblasts. Walking, jogging, dancing, and stair climbing are considered weight-bearing activities that support bone health. Resistance training—using free weights, resistance bands, or weight machines—has been shown in multiple studies to preserve or even modestly increase bone mineral density. Aim for at least 150 minutes of moderate-intensity activity per week, incorporating resistance training two or more days.

Smoking Cessation and Alcohol Moderation Smoking is an independent risk factor for bone loss and fracture. Alcohol consumption above moderate levels—more than one drink per day for women—is also associated with reduced bone density and increased fall risk. Addressing both behaviors is a straightforward but impactful step.

Fall Prevention For women with existing osteopenia or osteoporosis, preventing falls is as important as preserving bone density. Simple home modifications—removing loose rugs, improving lighting, installing grab bars in bathrooms—can significantly reduce fracture risk. Balance training exercises and regular vision checks are additional strategies worth discussing with your physician.

When Medication Becomes Part of the Plan

For some women on anastrozole, lifestyle measures alone are insufficient to offset bone loss, and pharmacological intervention becomes appropriate. Several classes of bone-protective medications are available and have been studied specifically in the context of aromatase inhibitor therapy.

Bisphosphonates such as alendronate (Fosamax), risedronate (Actonel), and zoledronic acid (Reclast) inhibit osteoclast activity—slowing the rate at which bone is broken down. Zoledronic acid, administered intravenously once or twice yearly, has been particularly well-studied in breast cancer patients receiving aromatase inhibitors and has demonstrated the ability to preserve bone mineral density in this population. Oral bisphosphonates taken weekly or monthly are a common alternative for women who prefer or require that route.

Denosumab (Prolia), a monoclonal antibody that targets a protein involved in osteoclast activation, is another option. Administered as a subcutaneous injection every six months, it has shown significant efficacy in preserving bone density in postmenopausal women with breast cancer receiving aromatase inhibitor therapy.

The decision to initiate bone-protective medication is individualized. It typically depends on baseline DXA T-scores, FRAX-calculated fracture risk, the expected duration of anastrozole therapy, and other patient-specific factors. This is a conversation to have directly with your oncologist, and potentially with a bone health specialist.

Building Your Care Team and Staying Engaged

Managing bone health during anastrozole therapy is a collaborative effort. Your oncologist oversees your cancer treatment and can initiate DXA referrals and bisphosphonate prescriptions. Your primary care physician or internist monitors overall health and can coordinate with specialists. An endocrinologist or rheumatologist may be consulted for complex cases involving significant bone loss or prior fractures.

Patient engagement is equally important. Keep a record of your DXA results over time to track trends. Ask your care team at each visit whether your bone health plan needs adjustment. Bring a list of all supplements and medications you are taking, since some—including certain antacids, corticosteroids, and thyroid medications—can affect bone metabolism.

Bone loss from anastrozole is a real and documented side effect, but it is not inevitable or unmanageable. With early screening, consistent lifestyle habits, and appropriate medical intervention when warranted, most women can protect their skeletal health without compromising the cancer treatment that anastrozole provides. The key is starting the conversation early and returning to it throughout the course of therapy.
