This means emphasizing whole grains over refined ones, limiting sugary beverages and processed snacks, and structuring meals around fiber-rich vegetables, legumes, and complex carbohydrates.

Consider the Mediterranean dietary pattern. Several studies examining dietary interventions in breast cancer survivors have found the Mediterranean diet — rich in vegetables, fruits, whole grains, legumes, fish, and olive oil — to be associated with favorable outcomes related to weight, inflammation, and metabolic health. Be mindful of alcohol. Alcohol contributes empty calories and may also influence estrogen metabolism in ways that are not fully understood in the context of AI therapy. Limiting or eliminating alcohol is a reasonable step for patients managing weight during anastrozole treatment.

Exercise as a Therapeutic Tool

Physical activity is arguably the most powerful modifiable factor available to patients experiencing weight and body composition changes on anastrozole. Resistance training — also called strength training or weight-bearing exercise — deserves particular emphasis. By stimulating muscle protein synthesis, resistance exercise directly counteracts the muscle loss that contributes to metabolic slowdown during AI therapy. Two to three sessions per week, targeting major muscle groups, is a widely recommended starting point.

Aerobic exercise remains important for cardiovascular health, caloric balance, and fatigue management. Activities such as brisk walking, cycling, swimming, or low-impact aerobics are accessible to most patients and carry additional benefits for mood and bone density — both of which are relevant concerns during long-term anastrozole use.

Patients who have not exercised regularly prior to diagnosis should consider working with a physical therapist or certified fitness professional with experience in oncology rehabilitation. Many cancer centers and community hospitals across the United States offer exercise programs specifically designed for cancer survivors.

When to Bring It Up With Your Care Team

Weight and body composition changes during anastrozole therapy are medically relevant, not merely cosmetic. Significant increases in abdominal fat are associated with elevated cardiovascular risk and insulin resistance, both of which warrant monitoring over the course of a five- to ten-year treatment course.
